Facing Illness, Injury or Disability
If you become unable to work due to illness or injury, Kraft’s benefits help protect your income. These benefits have three components: sick days for hourly employees/paid time off for salaried employees, short-term disability (STD), and long-term disability (LTD).

Contact your supervisor as soon as you know you will miss work.

If you expect to be out longer than seven calendar days, contact your disability case manager (DCM). If you are not sure who your DCM is, contact your local Human Resources Manager.

To report your disability claim, contact the disability claims administrator (DCA). The DCA will send you a Medical Authorization Form and Reimbursement of Overpayment Agreement Form to complete. Review, sign and return the forms within 21 calendar days of your first missed work day to prevent a lapse in disability pay. Also, give a copy of the Medical Authorization Form to your doctor (you must be under a doctor’s care to receive disability benefits).

If you are approved for STD (or Accident & Sickness) benefits and your disability is expected to last 26 weeks or longer, the DCA will coordinate the claim transition from STD to LTD (if you elected LTD).
