• Notify your supervisor you’ll be taking time off.

• Make any appropriate changes to your Kraft benefits within 31 days.

• Update your beneficiary information for your Kraft plans and update your personal information.

• If your dependent was enrolled in any Optional Protection Programs, benefits may be payable under Group Universal Life Insurance and, if death was accidental, Group Personal Accident Insurance.
