Prescription Drug Program through Caremark In-Network or Out-of-Network

(out-of-network reimbursement based on in-network price)

Retail Network Pharmacy
  • You pay: 30% per prescription/refill for up to a 30-day supply

    • Your minimum cost per prescription: lower of full cost of drug or $10 generic/$20 brand-name

    • Your maximum cost per prescription: $50 per prescription or fill

  • For maintenance drugs after first three fills: you pay 50% (minimum $10 generic, $20 brand name), which is not subject to $50 maximum cost per prescription and not counted toward the annual out-of-pocket maximum.

Mail-Order Service
  • Intended for prescription drugs you take on a longer-term regular basis (maintenance medications)

    • You pay: 25% per prescription/refill for a 90-day supply

    • Your minimum cost per prescription: lower of full cost of drug or $20 generic/$40 brand-name

  • Your maximum cost per prescription: $100 (per 90-day supply)

  • With FastStart you or your doctor can call Caremark at 1-800-875-0867 to place your order. And you can pick up your maintenance prescription at your local CVS pharmacy or have it mailed to you.

  • If you have questions about using the mail-order service, go to Caremark or call 1-866-KRAFT-RX (1-866-572-3879).

Annual Out-of-Pocket Maximum
  • You pay: $1,000 per person/$2,000 per family (separate from the medical maximum)

  • Expenses for maintenance prescriptions filled at retail for the fourth and subsequent fills do not apply to the annual out-of-pocket maximum.

Lifetime Maximum for Infertility Drugs
  • $15,000 (amounts previously paid under a Kraft Medical Plan apply to the lifetime maximum).
