Benefit Option Worksheet.
Once you make your Medical Plan election, you can't change it until the next
Annual Enrollment period unless you have a
qualifying change in status.
Using Network Providers
You get maximum benefits going to in-network doctors and hospitals because:
Kraft pays a larger percentage of the cost.
The cost is usually lower because rates are negotiated.
Reasonable and customary limits do not apply to in-network care because the
rates are already negotiated.
As always, you should verify Network participation with your provider and Member Services for both your doctors and hospitals each
time you need care.
What Is a Network?
A Network consists of doctors, hospitals, clinics and other providers that contract
to provide services at a negotiated price in return for getting more patients. When you use
Network doctors and providers, you receive higher benefits based on negotiated rates.
You do not need specialist referrals.
You can go to any doctor in the Network any
time — or you can see a doctor outside the Network. It's your choice each time you
need care. Remember, with out-of-network providers, you receive lower benefits.
If you want in-network benefit levels, you must determine that the provider is in-network.
It's important that you verify Network participation with your provider and Member Services for both your doctors and hospitals
each time you need care.
